As a Hospice Triage Nurse (RN) you will be called to care when you’re needed most. As part of Interim HealthCare, you’ll support a full range of patient services to bring comfort and dignity to our clients. What we offer our Hospice Registered Nurse Case Manager (RN):

  • Call Center Hours: Saturday & Sunday from 8am-8pm and Tuesdays from 5pm-midnight

• Provide the initial telephone assessment to determine the best solution to address the needs of the caller on weekends, holidays and after regular business hours.
• Consult with Hospice Medical Directors, Hospice Physicians and Palliative Physicians to meet the needs of the patient.
• Dispatch designated on-call field nurse to patent’s residence.
• Provide education and instruction on interventions to support effective management of the patient’s needs including pain and other symptoms as well as assessing the patient/family’s response to the interventions and treatment.
• Complete accurate and timely documentation of calls received, and interventions and education provided.

To qualify for a Hospice Registered Nurse Case Manager (RN) with us, you will need :

• Graduate of an accredited school of nursing.
• Licensed to practice as an RN (multi-state license required for PA, OH, IN). License is active and in good standing.
• Two years of recent hospice experience required.
• Triage experience preferred.
