Phoenix Home Care and Hospice is hiring compassionate and dependable nurses for night shifts, weekend shifts, and flexible scheduling options. We offer full time, part time, and PRN opportunities for LPNs and RNs who want to provide meaningful one on one care. Responsibilities
Provide one on one nursing care to pediatric and young adult patients in their homes
Deliver skilled, compassionate care to support complex medical needs
Monitor, document, and report changes in patient condition
Follow the established plan of care and communicate with the care team
Build strong, trusting relationships with patients and their families
Requirements
Current and active LPN or RN license
Pediatric experience preferred but not required, training provided
Strong commitment to patient centered care
Ability to pass all background and compliance requirements
