About the Role:

A community-focused healthcare organization is seeking an RN Medicaid Care Manager to support Medicaid patients with complex clinical and social needs. This role provides care navigation across the full continuum of care, helping reduce hospital readmissions, improve outcomes, and support vulnerable populations with high ED utilization.

You will work independently with strong autonomy, collaborate closely with care teams, and serve as a key advocate for patients needing additional support, education, and coordinated services.

Key Responsibilities:

- Manage chronically ill and high-risk Medicaid patients to improve health outcomes
- Provide care navigation, education, and self-management support
- Coordinate safe and effective care transitions across hospitals, clinics, and community settings
- Collaborate with ED teams and social workers on discharge planning
- Assist with follow-up appointments, medication access, referrals, and community resources
- Support high ED utilizers by addressing medical and psychosocial barriers
- Track and report care management metrics (LOS, readmissions, follow-up care, avoidable days)
- Conduct telephonic and in-person outreach using motivational interviewing techniques
- Identify and enroll new high-utilizer patients (6+ ED visits in 12 months)
- Advocate for patients and connect them with housing, transportation, nutrition, and other supports

Qualifications:

- Active RN license (SC or compact)
- Experience working with Medicaid populations preferred
- Strong care coordination, patient advocacy, and communication skills
- Comfortable working independently with vulnerable or marginalized groups
- Ability to travel locally for patient visits as needed
- Passion for public health and reducing ED overuse

Additional Details:
